Track Number in Shuffle All mode resets to 1 before all songs are played once

I never had this issue before, but now Poweramp randomly decides to reset the [track number] / [total number of tracks] displayed in the widget, resulting in some songs being played twice before all songs are played once.

I assume you mean in Shuffled playback? Is this during one session or do you exit the app and come back later to find this issue?

Yes I mean in shuffled playback. I am not sure exactly when it happens, but I found out about it when a specific track was played twice in a short span of time. Then I checked the widget and realized the track counter reset back to 1. I was not even close to playing my whole library once through.

So in one session, you didn't stop or exit the app and come back to continue playing later?

I didn't exit the app, merely paused the playback by removing my headphones from the audio jack.

That should have just caused playback to pause, rather than to re-shuffle the list.

The easiest way to check is to skip back a few tracks (as the playback history list is retained during shuffled playback). You will only be able to go back as far as the point where the shuffled list was first started though, so that will indicate where a reshuffle last happened.
